		Please attach a copy of your child's birth certificate. This is used as proof of age.
		Please attach a document as proof of your address. This document must not be more than 6 months old. Acceptable documents are utility bills such as gas, electricity, television and broadband. Printed online bills are acceptable. Mobile bills are not acceptable.
		Please attach NCSE eligibility letter if applying for a place in the Autism Class.
