		Please attach a utility bill or similar as proof of address.
		For Special Class (ASD) ONLY. Please upload any letter of recommendation for an ASD Class, assessment documentation and/or other relevant information that will support your child in primary school e.g. Autism assessment, AIMS support targets, HSE reports etc. Supporting documentation can also be emailed to office@scoilchoca.ie but please include your childs name and the application number.
